Please fill out this intake form to initiate booking an appointment. I will try to respond within 24 hours to your inquiry. If you feel your need is urgent, please note this in your intake form.
If you have insurance please follow the next step as well.
Step#2 - check your insurance coverage
Valley Woods Lactation accepts many insurance plans, including United Health Care (including UMR) PPO & HMO, Aetna, VA CCN - Optum East, PNOA - Provider Network of America, Multiplan and all CIGNA plans.
This will cover your lactation visits in full (a travel fee may apply when >20 miles from Intervale Scenic Vista, Intervale, NH)
If you do not have one of these insurances, I do accept out of pocket payment as well, upon completion of the visit. I can supply you will a SUPERBILL (itemized receipt of services) you can submit to your insurance and you may be reimbursed.
